Working Toward Shared Goals

Making advance care planning part of everyday healthcare takes collaboration across clinical care, public policy, technology, and community education. Our memberships connect us with organizations working to improve care for people and families and make health information accessible across care settings.

We participate in these communities to share practical experience, learn from others, and contribute to discussions about advance care planning and interoperability. Together, we’re working toward a healthcare system where patient goals of care and treatment preferences are documented, available, and used to guide care.
