Advance care planning (ACP) is a core component of post-acute care. As quality measures tighten to align care with stated preferences, high-quality, interoperable ACP is increasingly in the spotlight. For many acute-care providers, the growing emphasis on care alignment requires renewed attention. ACP programs frequently don’t operate at their highest potential or achieve their primary objective: helping individuals avoid unwanted or non-beneficial interventions and aligning care more closely with their documented treatment preferences. Too often, documents are inaccessible at critical moments of decision-making. When clinicians cannot reliably retrieve treatment preferences, care teams may default to high-intensity interventions that do not reflect patient wishes. Fragmented processes further increase the risk of delivering care that patients may not have chosen. This session will present a structured framework and practical tools that organizations can implement to strengthen ACP programs and improve the consistency, accessibility, and reliability of documented treatment preferences across care settings.
